What is a template?
A template is a structure that the generative AI will use as a basis for creating the note. For Tali, it isn't only the structure, but also the instructions we provide to ensure that the note is generated to suit your expectations.
Tali offers multiple templates, designed to suit different situations. This article is about how to use them.
How to select a particular template
Once you start recording with AI Scribe, you will see a template drop down in the screen. Click on that dropdown to select which template should be used to create the note.

SOAP Note
Used mostly in primary care, the SOAP note (Subjective, Objective, Assessment, Plan) provides a problem-focused summary of a patient's visit.
SOAP Note Multiple Issues
Some patients attend with more than one issue. This template organizes the information for the various issues appropriately within a single SOAP note.
SOAP - Merged A&P
Use this SOAP template when looking to have the Assessment and Plan of the SOAP combined into one section to better customize Tali to your note taking preferences and style.
Comprehensive SOAP
A more comprehensive SOAP template broken down by: HPI, Past Medical History, Surgical History, Medications, Allergies, Immunizations, Review of Systems, Social History, Family History, Physical Examination, Lab Results, Imaging Results, Assessment and Plan
Consult
Used mostly by specialists, a consult note is written with the referring clinician in mind, summarizing the patient visit.
Consult Letter - Detailed
Use the Consult Letter - Detailed when responding to a referring clinician about a referral request that was sent to your office regarding their patient. The detailed template is broken out into multiple sections, including HPI, ROS and more.
Addiction Note
This template catered towards addiction specialists and is is broken down into the following headings: Presenting Problem, HPI, Past Medical History, Surgical History, Medications, Allergies, Immunizations, Review of Systems, Social History, Family History, Vital Signs, Mental State Examination, Physical Examination, Lab Results, Imaging Results, Diagnosis and Assessment, Discussion, Plan.
Mental Health Progress Note
This note is for mental health specialists who are documenting follow up appointments. The headings are as follows: Presenting Problem, Current Medications, Update, Therapy, Plan, Medication Changes, Refilled Medication
Patient Summary
This note summarizes the encounter and what was discussed, but is written for the patient to read. Thus, it tends to use lay language, and has a more informal structure.
Referral Letter
Use the Referral Letter template to refer a patient to another clinician for care or advice. Save time referring patients to specialists for further evaluation or treatment of a specific condition.
Physical Therapy SOAP
This template is similar to Tali's existing SOAP note, but designed for Physiotherapy encounters. It includes the regular SOAP headings and the following additional headings: Chief Complaint, Problem and Goals with long and short term goals and achieve by dates.
Psychiatry Consult Letter
The Psychiatry Consult Letter is similar to Tali’s existing Consult Letter template, but geared towards a Psychiatry encounter.
Psychiatry Consult Letter - Adult
This template turns the patient conversation into a thorough consult note, covering several key areas, including: Reason for Consultation, HPI, Past and Current Patient History, Examinations, Diagnosis, and Recommendations. You may use this Template for the majority of mental health conditions, including addiction, anxiety, PTSD and more.
Psychiatry Intake Note (Child and Youth)
Use this Intake Note to bring structure to your psychiatry intake processes. This template will help you prepare a detailed summary of your patient encounter.
Social Workers Notes
A template for Social Workers based on Social Work Theory, with notes broken down by: Identified Concerns, Interventions, Session Focus, Progress and Plan
Pediatrics - Well Child Care Visit
With this template, clinicians will be able to document a comprehensive visit including a complete physical exam, and key visit elements such as Chief complaints, HPI, ROS, Birth history, Immunizations, Development, Growth parameters, Medical history, Surgical history, Family history, Social history, and more!
Creating Multiple Documents or Notes from One Patient Conversation
Often one document from the patient's visit is enough, but sometimes you will want to generate multiple documents or notes from the single encounter.
With Tali, you can now automatically turn one patient note, such as a SOAP note, into other formats, including: SOAP Multiple Issues Note, Patient Summary or Consult Note. Let’s show you how.
How to generate multiple notes from a new patient conversation
Go to the Home tab in Tali
Click the ‘AI Scribe’ button to record a new patient conversation
At any point during the recording, pick the first template you’d like to use
Once you have finished recording your patient conversation, click ‘Done’. As usual, your note will be generated
Next, click the ‘Generate’ button located under your Encounter title
Choose a template from the list available to create your next note
Once generated, edit, copy or save your new notes to your EHR as you normally would
You can do this as many times as you need.

Click on the History tab in Tali
Choose the Patient Encounter you’d like to generate a new note type from
Underneath the Encounter’s title, click the ‘Generate’ button
Choose from any other templates available to create additional notes
Once generated, edit, copy and save your new notes to your EHR as you normally would
