Example Note with Tali

This article with provide an example of how Tali takes your transcript to transform it into a note

Getting started with Tali is easy! From logging in to creating your first note, this article will guide you through each of these steps so you can start saving time right away!


Logging in

  1. Navigate to your version of Tali (E.G. Integrated into your EMR, Web App, Chrome Extension) and open Tali.

  2. (Except for most integrations) Select the Sign into Tali option.

  3. Enter your email address which was used to sign up to Tali, and select Continue.

  4. You will receive a 4 digit code to the email you just entered. Navigate to your email app, retrieve the code and enter it where prompted.

  5. Success! You're now logged in to Tali. Let's start with your first encounter encounter...

Beginning the recording

To begin recording your encounter, hit the Start Recording button

From here, you can record your encounter naturally, and Tali will transcribe the conversation and convert it into a note of your choosing!

  1. Select the Start Recording button.

  2. In the Select Template dropdown, choose SOAP Multiple Issue.

3. Open the example transcript below, copy and paste the text in to the Add additional context field.

Example Ear Infection and Ankle Transcript

Hi, come on in. You must be Jordan?

Yep, that’s me.

I’m Dr Byfield—good to meet you. How’s your day going?

Not bad, honestly. Bit tired. This ear’s been driving me nuts.

Ah, sorry to hear that. Left ear, right?

Left. It’s been hurting for a couple of days now. At first it was dull, but yesterday and today it’s been sharper. Kind of feels blocked. I even got a bit dizzy when I stood up this morning.

Okay. Any discharge from the ear? Fluid or anything?

Yeah, yesterday there was some yellowish gunk. Not a ton, but it was there.

Any fever?

Last night I was sweaty, kind of burning up. Didn’t take my temp though.

Gotcha. Did you have a cold recently?

Yeah, last week. Just a regular cold, runny nose, bit of a cough. Thought I was over it.

That’s helpful. Alright—before I take a closer look, let me ask: any medication allergies?

Yeah, sulfa drugs. I found out after I broke out in hives from Bactrim a few years ago.

Good to know. Any other medical conditions?

I had asthma as a kid. Mostly under control now—I keep an inhaler around just in case, but haven’t used it in over a year.

Perfect. Thanks for the heads up. Mind if I do a quick exam?

Go for it.

Starting with your temperature… That’s 38.1°C—so, mild fever. Now I’m going to take a look at your ears. Just tilt your head a bit for me—thanks.

The left ear canal’s red and swollen… a little discharge visible… eardrum looks cloudy and slightly bulging. Yep, that’s classic for a middle ear infection. Right ear’s all clear.

I’ll just feel under your jaw now for lymph nodes… okay, some tenderness and mild swelling. That lines up.

Last thing—open wide and say “ahhh”… no tonsil swelling, little post-nasal drip. Looks like this started as a viral cold and turned bacterial in the ear.

Makes sense, yeah.

I’ll prescribe Amoxicillin 500 mg, three times a day for 7 days. It’s a penicillin antibiotic that should clear things up. You’ll probably notice improvement within 48 hours.

Okay, sounds good.

Since you mentioned the sulfa allergy, it’s good you’re not allergic to penicillins, just for the record.

Yeah, penicillin’s fine.

Alright—anything else bothering you?

Actually, yeah. My ankle’s acting up again—right side. I sprained it playing soccer about six months ago. Did physio back then and it helped, but it’s been flaring up again lately.

Still playing?

Trying to. But I can’t go full speed. Hurts afterward, and I feel like I’m favoring it a bit.

Let me take a look.

I’m checking for swelling—none visible. Now I’m going to move it through some motions—let me know if anything’s painful.

Flex up… good. Point down… any pain?

A bit, yeah.

How about side to side?

That’s okay.

Bit of tenderness over the outer ligament. Feels stable, no major weakness, but sounds like there’s some lingering inflammation or scar tissue.

I’ll refer you for another round of physio—focus on strength and balance work. That’ll help prevent it from turning into a chronic issue.

Appreciate that.

No problem. I’ll send the prescription for the antibiotics to your pharmacy, and the physio referral will go through today. Let me know if anything changes or gets worse, alright?

4. Your transcript has now been converted into a SOAP Multiple Issue note!

5. You can now generate a different template using the same transcript. Select the Generate button and choose Referral Letter.

6. Tali will now generate a Referral Letter using the same transcript!
